Aged Care Referrals

Aged care provides support for older people to help them with everyday living and other needs. These government funded home care packages are in place to ensure that those assessed as requiring extra support, can receive financial support to achieve their goals. 

As health professionals, our services ensure that older Australians are provided with holistic services that allow them to live in their homes and maintain their quality of life and independence. Home care package funds are able to be used to fund these services and our organisation will liaise with your home care provider to receive approval. 

Goals of treatment:

• Increasing strength to continue to complete daily activities

• Improving balance and mobility to reduce the risk of falls

• Dietary review to ensure peak nourishment

• Improve psychological wellbeing

• Support with medical needs
